

You and your athlete electronically sign-off on these documents via the parent’s and athlete’s Final Forms account. If you have any questions about using Final Forms, refer to the Parent Registration Guide. Registration: The Emergency Medical Information, Travel Permit, Athletic Handbook Acknowledgement, and Concussion Acknowledgement are no longer paper forms. Even if your student’s current physical paperwork does not expire until after the new school year begins, or if you completed paperwork for a fall sport, please register electronically on Final Forms. We are asking that ALL student-athletes (grades 7-12) use FinalForms. Please click on Final Forms in the black box below to open an account: All forms are to be completed by July 30, 2022 if your child is planning on playing or trying out for a Worthington Cardinal Fall Sport’s Team. Once your student’s physical form is received by the athletic department, we will input the expiration date into FinalForms and then you will receive automatic notifications when your student’s forms need to be updated. The physical form must be completed by the doctor and by law we are required to have a signed copy on file.

The ONLY form that you are still required to actually print, complete, and file in the Athletic Department is the actual OHSAA Physical form. FinalForms is immediately available for all Worthington Cardinal Middle School student-athletes. The Worthington Athletic Departments have partnered with FinalForms, an on-line program which will enable you and your student to complete and file MOST athlete forms electronically.
